Let us know your sizes and quantities here by March 27, 2010. If you've already paid for your shirts with your entry fee check, just fill out this form and you're done. If you haven't paid for shirts yet, you'll need to tally what you owe and mail a check. Remember that your first 5 shirts are FREE, so don't count those in your total! Checks should be payable to SDA-CANSTRUCTION, mailed to:
